Help: Dogs ears always infected

TIA for any advice given!

My German shepherd, big boi Hugo, for the last 2.5yrs has been getting ear infections on and off. I’ve been cleaning his ears with epiotic regularly, however the debris just doesn’t stop.

I’ve taken him to the vet multiple times where they originally put him on prednisone plus prescribed Easotic ear suspension. They said he had bacteria and yeast in his ear, and a huge amount of wax, which was expected. The meds work temporarily but once the prescription runs out his ear is fucked again within weeks.

I’ve put him on a rotation of antihistamines which have mildly help, but doesn’t fix it. Our last vet visit he was given cytopoint injection which obviously stopped his itch, but the gunk just continued.

He has been in for two ear washouts now, where they have pulled out huge amounts of wax and debris.

The vet has said that the next step is to see a dermatologist, but I can’t keep forking out these large bills.

We have tried changing his diet a few times, but haven’t noticed any significant changes with his ear. He’s currently on Purina sensitive skin and stomach, as we were told this could help. I’m also using sonotix wax removal to get big chunks out, sometimes slug looking pieces come out.

If anyone has experienced something similar, or has any advice it is greatly appreciated.
