One year old dog still has accidents in the house

I thought dogs are potty trained by now?! I’m at my wits end. I had my carpets cleaned and I still find potty accidents. Mostly pee but sometimes poop. I know more when it’s time to poop so I get her outside. She doesn’t ask to go she just goes and it’s so frustrating. When I go outside with her on a regular basis I tell her to go potty and when she does, I say “ yes” and give a treat. I watch her but at her age she hates the kennel. When I leave I leash her to the couch and she prefers that. I have kids and a busy life so it’s hard to watch her constantly and I take her out regularly- why isn’t she getting it? I also have a bell at the door so she can tell me she needs to go, but she mostly uses it to go out and play. When will she understand to not go inside? She doesn’t go at night at all so I know she has bladder control.
